Medical History Report
CGU seeks to provide a safe and healthy environment to all on campus and encourages students to take an active role in their own health. All students are required to submit the CGU Medical History Report with proof of vaccinations and immunizations (see required list below). To protect your privacy, all medical forms must be uploaded directly to the American DataBank Upload Portal at http://cgucompliance.com/.
Tips for Uploading Your Documents
- Use a laptop or desktop computer
- Go to http://cgucompliance.com/ and select NEW USERS Create an Account
- Complete required information and click Create Account and Proceed
- Go to your email – check the JUNK folder – and click on the link to activate your account
- Log in via Existing Users and select Get Started
- Choose the one option in the dropdown, and select Load Packages
- Choose Claremont Graduate Rules Package and select Next
- Click Upload Documents in the top right corner. Attach your Medical History Report, separate Immunization record, and any Request for Exemption
- Enter dates of Immunizations/Screens for those required: MMR, Varicella, TDAP, Covid, TB.

Students must provide proof of vaccination, or approval for an exemption, at least 2 weeks prior to visiting campus. Students must provide proof of vaccination, submit a request for medical or religious exemptions, or certify that they will not visit campus.
- Upload your completed vaccination card to the American DataBank Upload Portal;
- Request a medical or religious exemption: complete and submit the Request for Exemption Form to the American DataBank Upload Portal; or
- Certify that you will not visit campus: complete and submit the Affidavit due to Remote Work/Study.

*Students who do not comply with the policy may have a registration hold added to their student account, preventing their ability register for courses.
- Please visit your medical provider or physician to determine if you live or work in a high prevalence area for tuberculosis, or a high-risk area. This determination should be made and confirmed by your medical provider or physician.
When does the Medical Record and Immunizations Policy go into effect?
- CGU’s Medical Report and Immunization Policy requires all students submit a tuberculosis screening. Beginning fall 2022, CGU has added the following vaccinations and immunizations to the required list: MMR, Td or Tdap, Varicella, and Covid-19. Students who do not comply with the updated policy may have a registration hold added to their student account, preventing their ability register for spring 2023 courses.
How did CGU determine which vaccinations and screenings to require?
- We reviewed the Centers for Disease Control and Prevention (CDC) adult guidelines for vaccine recommendations, along with the American College Health Association recommendations. Additionally, we made comparisons of other colleges, including the University of California (UC) system.
I do not have access to my childhood medical records. What should I do?
- Adults who lived in the US, and were treated within the US should have access to their personal medical records through their health provider(s), and/or state or federal (military) health departments. For more information, please contact your health provider and/or state health department.
I did not receive vaccinations as a child. What should I do?
- The required vaccines are available to teens and adults who did not receive vaccinations as children. The expectation is that each student completes the required vaccinations and immunizations prior to coming to campus.
I have concerns regarding the safety of vaccines and have not been vaccinated. What should I do?
- We strongly encourage those who are concerned about the safety of the vaccines to get information from a reliable source, such as the CDC.
I had an allergic reaction to a vaccination. Am I still required to be vaccinated?
- Students who submit a completed Affidavit for Exemption to Vaccination form and medical provider documentation may be eligible for an exemption.
Is there a process to request a personal / religious belief exemption from the policy?
- Students who submit a completed Request for Exemption Form form may be eligible for an exemption.
Where can I receive needed vaccines?
- Vaccinations are widely available through the county and local pharmacies. Students can visit the Los Angeles County Public Health Department website to learn more.
How can I request my health insurance cover some, or all of costs associated of my vaccinations?
- It is recommended that students contact their health insurance provider prior to their appointment to verify covered / eligible medical expenses and if necessary, request how to submit claims reimbursement.
I cannot afford to have vaccinations completed prior to arriving on campus. Are there free or low cost options?
- Many domestic health insurance plans will pay 100% for the vaccines that are on the required list. For those who are uninsured, underinsured or their health insurance plans do not cover the cost, we encourage you to check if you’re eligible for Medi-Cal. In many cases, local health departments may provide vaccines at a reduced price. For more information, please visit:
This policy is being implemented during my last semester of enrollment. If I am unable to complete my vaccinations, will I be prevented from participating in commencement?
- Beginning fall 2022, all incoming and continuing CGU students will be required to adhere to the Medical Record and Immunization Policy. Please contact the Dean of Students Office if you have additional questions about this policy.
